customer when service is requested. After we receive this form, we will issue
you a Service Number and shipping instructions. If you need a Laptop box,
we'll be happy to send one to you.

I fully understand
that I, the owner of this computer, have backed up all information on the
hard disk Drive.
I understand
that the data on this computer is not the
responsibility of M Rugged Mobile Technology and may be lost during the repair
process without my prior knowledge and waive any legal action against this
company.
I understand that M Rugged Mobile
Technology is not responsible for items not claimed within 90 calendar
days of the third notification attempt. I also understand that M Rugged Mobile
Technology may place a lien or contract a collection agency to recover any
cost incurred to M Rugged Mobile Technology.
I understand
that there is a minimum charge of a
$105.00 Diagnostics bench fee for all out of warranty work. This is in addition
to any other parts, labor and shipping charges. We will provide a written
estimate of repair cost after completion of diagnostic testing.
I understand
that any software related issues that are
found to be the cause of the machine’s failure is not covered under any standard
or extended manufacturer warranty.
I understand
that some repairs may take up to 14
business days dependent on parts availability from panasonic. Our Typical
repair takes no more than 3 business Days excluding shipping
